Discord admits AI moderation bug, unfairly bans users for innocuous images

Discord has admitted that it incorrectly banned more than 8,000 users over the past two months due to a bug in its AI moderation system after innocuous images including spreadsheets, chessboards, game textures, and transparent white and gray backgrounds were incorrectly flagged as harmful content.

The company confirmed that the issue has been affecting accounts since May, and that it banned an additional 200 users over the weekend until its team identified and fixed the issue. All affected accounts are currently being restored.

The incident highlights one of the growing challenges surrounding AI-assisted moderation, as many platforms increasingly rely on automated systems to identify illegal or fraudulent content at scale.

In a detailed thread about X, Discord explained that its automated safety system works by checking uploaded content against a database of known hazardous substances. While the technology is designed to detect illegal content, the company acknowledged that false positives can occur. Human moderators reviewed the content, but the bug caused the system to immediately ban the affected accounts.

“We are working on better safety measures to prevent something like this from happening again,” the company said.

Across X and Reddit, users are claiming to have been permanently suspended simply for uploading an image containing a square grid pattern. Some users have speculated that Discord’s AI moderation tools are becoming increasingly sensitive to grid-like patterns. This is because these tools have previously been used in attempts to hide or disguise NSFW and child exploitation content from automated detection systems.

Affected users have expressed their displeasure on social media, with some arguing that a permanent account ban based solely on automatic detection could have serious consequences, especially for users who rely on Discord for work, gaming communities, and long-distance social connections.

“Losing a Discord account to something unfair like this is extremely devastating and can severely impact users, and millions of users are affected by erroneous AI bans every day. This needs to stop,” one X user wrote.

Discord is not alone in facing moderation issues with automated systems. Last year, users on Instagram and Facebook groups reported widespread unexplained suspensions that many believed were caused by AI moderation systems. Users pointed to automation as a likely cause, but Meta never officially confirmed whether an AI error was the culprit. Now, Meta’s oversight committee is calling for more transparency.

Tumblr also faced complaints last year from users who said their accounts were being suspended en masse with no clear explanation.
